At Park Road Surgery in Teddington we offer Medical examinations for insurance purposes, sport, private schools elderly drivers’ licences etc. are done at special times. Please check with the receptionists. There is a charge for this service, payable on the day of the examination.

Please let us know if you cannot keep your appointment.

Forms

There is a charge for the completion of forms for private health insurance, holiday cancellations, etc. Forms for completion should be left with reception who will pass them to the doctor. Please do no make an appointment with the doctor for the sole purpose of asking them to complete these forms; in any case the forms would not be completed during the consultation and you would need to return to the Surgery to collect the completed form.

Please note that we do not have the facility to pay at the surgery with credit or debit card. If you need to make a payment to the practice please pay in cash or cheque (made payable to ‘Park Road Surgery’).

Request for letter to school regarding sickness during exams

We have been advised by the BMA that exam boards do not require a letter from a child’s GP, following sickness during exams. In the event that a school requests such a letter then we will send them the reply attached, detailing why it should not be necessary. If you still require a letter to the school to detail sickness absence during your child’s examinations then there will be a charge of £70, as this is outside the remit of NHS work.
